JOB DESCRIPTION:
• Typing of all internal medical reports.
• Typing of medical certificates of patients.
• Ensure that the typed reports are correct.
• Co-ordinate with the Front Office Executives, Doctors for the Discharge Summaries, Radiology.
• Ensure that the reports / certificates are all typed within time.
• To get the reports / certificates / summaries checked from the Doctors.
• Maintain a good relationship with all the departments.
• To formulate the Process Improvement programs and carry on Quality Initiatives in the department.
• To perform all other jobs as may be assigned due to exigencies of work.
JOB DESCRIPTIONS
1. Take the history and perform an examination of the patient. Male doctors shall ensure the presence of a nurse or female colleague if examining a female patient.
2. Document history & physical examination within 24 hours of admission
3. Write daily progress notes at least once in 24 hours and more frequently if patient's condition requires more frequent observations.
4. Write all orders in legible writing in the “Physician order sheet”, after confirmation with a senior colleague or consultant.
5. To perform procedures of intravenous vascular access lies, urinary catheter, and Ryle's tube independently.
6. Carry out invasive diagnostic procedures such as paracentesis & thoracocentesis according to the directions of the consultant and under strict supervision.
7. Take appropriate blood samples if required.
8. Follow up on investigations sent and communicate results to the consultant. Document such communications and the resultant necessary orders.
9. Assist his/her consultant in the OPDs.
10. To be actively involved in CCU/HDU/SICU /PRE-ANGIO care of the CARDIAC patients.
11. To complete the all necessary medical documentation pertaining to the patient care including doctor to doctor hand over.
12. Do daily dressings of wounds, as per directions of the consultant.
13. Send appropriate referrals and communicate with consultants.
14. Ensure completion of patient records at the time of discharge including discharge summaries.
15. Respond promptly to any nurse’s urgent request for help in caring for any patient.
16. Respond promptly to Code Blue announcements.
17. Prepare discharge summary for patients under their care.
18. Participate in the Hospital’s quality improvement plan.
19. Participate in clinical and non-clinical training programs organized by the Department and the Hospital.
20. Attend to all the emergency calls from the causality & wards and then inform the consultant.
21. Take night rounds for all sick & post procedure patients.
22. Resuscitation through Risk Stratification for all ages. (Basic Life Support).
23. Participation in Continuous Quality Improvement.
24. To formulate the Process Improvement programs and carry on Quality Initiatives in the department.
25. To perform all other jobs as may be assigned due to exigencies of work.
